Case Manager II - Clinical Resource Management - PT and FT

Requirements:
Minimum 2 years experience in case management, discharge planning, and/or utilization review within the last 5 years. Bachelors degree with 3 years clinical experience in an acute or home health setting; or RN with 5 years clinical experience in an acute or home health setting. Valid CA RN license. Quality management, community health or home health coordination experience desirable.
Ability to anticipate, prioritize and follow through on responsibilities. Ability to adapt to a changing health care environment, and department priorities. Assertive and creative in problem solving, and systems planning and management.
Possesses excellent service focused skills. Participates as a team member or assumes a team leader role as assigned for daily work as well as performance improvement efforts within the department.
Ability to read and interpret medical records and hospital policies and procedures. Ability to write appropriate detailed continuing care notes in the medical record. Ability to communicate effectively with patients, families, physicians, staff, and community providers/agencies.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals and apply to practical situations.
Ability to define issues/problems, collects data, establish facts and draw valid conclusions. Ability to interpret technical/clinical instructions, and deal with abstract and concrete variables.
