Position Details
Will analyze and review the patient charts and assign codes for hospital ambulatory, emergency, outpatient and inpatient services rendered in the hospital setting. The professional coders will be required to adhere to strict productivity and quality standards. Inpatient Hospital coders must have either a certification as a Certified Coding Specialist (CCS), Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA) with five (5) plus years experience.
The Services to be provided are as follows:
Reviewing patient medical records (electronic health record) and health information data in order to classify and assign numeric codes with respect to diagnosis, procedures and other medical data. Such review and coding shall include, without limitation:
· The examination and interpretation of health information, documentation, data integrity, and medical records, including a verification of completeness, accuracy and proper entry into computer systems and other records;
· The evaluation of all relevant factors required under the coding standards promulgated by various standard setting bodies and payers;
· The utilization of knowledge and exercise of professional judgment regarding medical terminology, disease processes and pharmacology in the assignment of numeric codes under the applicable coding systems, including, without limitation, the International Classification of Disease (9thRevision) Clinical Modification (“ICD-9-CM), Current Procedural Terminology (“CPT”) and the Health Care Financing Administration Common Procedure Coding System (HCPCS) and standards recognized under the National Correct Coding Initiative, developed for CMS for use by carriers with respect to claims submitted relating to physician or other services reimbursed under Part B of the Medicare program; and;
· Abstract required patient demographics and other clinical data as required or as needed per department policy and the state of California (OSPHD);
· Such other procedures and services incident to the foregoing and/or as may otherwise requested by KP Customer.
Requirements
Facility coders must have either a certification as a Certified Coding Specialist (CCS), Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA) with five (5) plus years experience.
Demonstrated ability to understand the clinical content of a health record, including the most complicated records. Must also be able to communicate with physicians in order to clarify diagnoses/procedures and sequencing of diagnoses. Ability to demonstrate knowledge of and utilize auditing skills related to coding quality and compliance
